Recovery from addiction is a progressive, evolving and ongoing healing process which is facilitated by an effective aftercare or follow up plan. After care plans are essential for all the chronic conditions like Substance Use Disorder (Addiction) and this part of the treamtment comes after the client completes the inpatient treatment procedure. When the client is discharged after having an inpatient treatment services, they experience stress, cravings, triggers, emotional instabilities and other daily life challenges and difficulties. These set of challenges and conflicts increase the risk of relapse to the maximum, specially in the early stages of recovery. The follow up plans provide essential emotional/mental care, support and assistance to the client to work through their recovery and healing in the outside world. The follow up plan of every client is developed according to their needs and problems.
